Nama badan dan alamat yang masih berlaku secara jelas.
Nama pemohon dan jabatan pada badan atau organisasi.
Jenis barang/jasa yang dipromosikan.
Mekanisme dan teknis penyelenggaraan undian.
Teknis penentuan pemenang.
Jangka waktu dan wilayah penyelenggaraan.
Tempat dan tanggal penyegelan kupon/sarana kelengkapan hadiah (untuk undian gratis berhadiah langsung).
Tempat dan tanggal penyegelan dan penarikan (untuk undian gratis berhadiah tidak langsung).
Daftar dan jenis hadiah (secara rinci dan jelas mengenai jenis, jumlah, merek/tipe, dan tahun pembuatannya dengan mempertimbangkan nilai guna/manfaat hadiah tersebut).
Tanggal dan cara pengumuman hasil penarikan undian melalui media massa.
